1.) Inserts: Pillow inserts are not included. We recommend crateandbarrel.com for inserts (free shipping, good prices, all the correct sizes). For a rectangle get a 15"x22" insert. For squares get one 2" larger than the pillow case. So if you're buying an 18" pillowcase, buy a 20" insert. We like the down alternative and also the feather down options. Links here.
